Did you know that insurances cover many of the home health services we provide?

If you or your loved one is in the hospital or receiving therapy at an inpatient facility, speak with the discharge planner about home health services. If you qualify, they will contact us. Our Care Transition Coordinator will visit with you in the facility and collaborate with the facility’s discharge team to ensure that the home care you need is arranged before discharge.

You may also qualify for home health services without a recent hospitalization or inpatient stay. If you have a change in your medical condition or an injury, we can help. Speak with your doctor about receiving home health services. If your doctor feels that you would benefit from nursing or therapy, he/she will contact us and provide all the information needed to set up services.
